Provincial Fund Guidelines

The purpose of the Provincial Fund is to provide financial support through grants to assist Member Groups with activities which directly improve the quality of life and well being of persons with cerebral palsy and other disabilities in Ontario.

ASSISTANCE PROVIDED

These grants are intended to assist the Member Group with short-term activities, one year duration or less, that can not be funded in full from other sources.

ELIGIBILITY

Eligibility for grants from the OFCP Provincial Fund is limited to full Member Groups of the OFCP who have maintained their membership in good standing as per OFCP criteria for the two years immediately prior to the grant application. The Membership criteria established by the OFCP Board of Directors are as follows:

1. Having a representative member in good standing that is:

a. Nominated for election to the OFCP Board of Directors, and/or
b. Appointed and approved as a voting delegate, and/or
c. Serving on the OFCP Board of Directors


IMPORTANT

The removal of the previously required provisions pertaining to involvement in either the Accreditation Ontario Outcomes Program or the OFCP Standards program, should not be construed as a lessening of support for either program. This will now enable all member groups to apply for grants to cover costs required to purchase the services of either Accreditation Ontario or approved, qualified consultants in the Standards field.


APPLICATION

Application for a grant from the OFCP Provincial Fund must be made to the Provincial Fund Committee of the OFCP. The application must be on the approved form (or follow the format outlined below), and include the following:

1. A statement of the intended use of the funds outlining how this project will achieve significant results for people in your community. Plus a detailed action plan describing current status and measurable goals including the steps necessary to achieve these goals.

2. A detailed budget for the project, including the amount of funding which has, or will be requested from other sources, plus all pertinent financial information. Note: preference will be given to applications where a portion of the funding comes from other sources.

3. Documentation which demonstrates that full funding from the other sources listed in the application will be available.

4. A certified copy of the Motion or Motions of the Board of Directors of the applying Member Group, approving the project, and authorizing submission of the grant application.

5. A copy of the most recent Audited Financial Statement, as well as a copy of the most recent internal financial statements.

6. Proof of membership in OFCP.

ADMINISTRATION

The OFCP Provincial Fund will be administered by the Provincial Fund Committee. The following procedures shall be used:

1. Allocations to the OFCP Provincial Fund shall be made by the OFCP Board of Directors at the end of the OFCP’s fiscal year. Allocations shall be made only from funds which are determined to be excess to those required to maintain the ongoing operations and programmes of the OFCP.

Any monies that were not allocated and were remaining from one year’s Fund will be added to the following year’s disbursement total.

2. All applications for grants must be received for review by the Provincial Fund Committee by December 1st in order to expedite the approval process. Activities for which grants are approved are expected to be completed within the calendar year that follows.

3. The Provincial Fund Committee shall review all applications received, and shall make recommendations regarding allocations to the Board of Directors no later than March. Grants may only be made following the approval of the OFCP Board of Directors.

4, In determining the priority of eligible grant applications, the following will be considered:

a. Does the project have a demonstrated need, and does it, or will it, benefit persons with cerebral palsy and other physical disabilities in keeping with the purpose of the OFCP Provincial Fund? If yes, please explain.

b. Is there a significant element of consumer involvement and direction in the project? If yes, please explain.

5. The grant payment shall be made on a schedule as determined by the Provincial Fund Committee in consultation with the applicant.

6. Member Groups which receive grants from the OFCP Provincial Fund must enter into an agreement with the OFCP which details their obligations under the grant. Progress reports will be required as the agreement. The letter of agreement will include a schedule for progress reports.

7. The Provincial Fund is not set to cover wages, salaries or work contracts of existing staff or new employees.

8. All decisions reached by the OFCP Provincial Fund Committee and approved by the Board of Directors are final.
TERMINATION / RECOVERY OF FUNDS

Eligibility for future grants will depend, in part, on satisfactory fulfillment of previous grant agreements. Grant funds utilized for purposes other than those detailed in the application and the grant agreement will be subject to recovery by the OFCP. If a group fails to fulfill the terms of the grant agreement they may be subject to legal action pending recovery of their grant.
