Provincial Fund Grant Application Form


The purpose of the Provincial Fund is to provide financial support through grants to assist Member Groups in Ontario with activities which directly improve the quality of life and well being of persons with cerebral palsy and other disabilities.

GRANT APPLICATION FORM

Applicants must have maintained their membership in the OFCP in good standing during the last two years.

Organizations applying for grants must include with their completed application the following:

• A certified copy of the Motion, or Motions, authorizing the request for funds by your Board of Directors

• A copy of your organization’s most recent Audited Financial Statement*

• A copy of the most recent internal Financial Statements

• A statement and/or documentation verifying that there are no outstanding debts, encumbrances or agreements that might make claim to the grant funds once they have been transferred.

(*Note - if an external audit of the accounts of the Member Group has not been conducted, then a statement to that effect, together with a copy of the most recent un-audited annual financial statements, must be provided).

Member Groups which receive grants from the OFCP Provincial Fund
must enter into an agreement with the OFCP which details their obligations under the grant.

All projects which receive funding through a grant from the OFCP Provincial Fund
shall be required to submit progress reports for the first five years
on a schedule as determined by the Finance Committee.
Further reports may be requested.
